The John A. Hartford Foundation announced that it was renewing funding for the CaRe-Align health care project for three more years for $3,889,741. The project was instituted by an 18-month planning grant from the foundation, in partnership with the Patient-Centered Outcomes Research Institute.
Dr. Mary E. TinettiThe CaRe-Align project is led by Dr. Mary E. Tinetti, the Gladys Phillips Crofoot Professor of Medicine (Geriatrics), and Dr. Caroline Blaum of New York University. It seeks to realign care around older patients’ self-identified health goals. The model also aims to improve health outcomes and lower care costs for this population, which is the major user of health care services in the country.
In this next phase, the model will be implemented and tested within ProHealth, a large primary care group serving the state of Connecticut. The three objectives for the prototyping phase are: implementing patient goals-directed care in practice; producing preliminary evidence for this model of care; and building awareness and demand through stakeholder engagement.
